Youth Permission Forms
Both of these forms are required to be filled out by a parent/guardian for all students who are participating in youth events with the Allensburg Church of Christ that take place outside of the church property. A paper form can be downloaded and printed, or an electronic form may be filled out online. Once forms are filled out, they do not need to be filled out again for subsequent events unless specifically indicated by the staff of the Allensburg Church of Christ.
